授業の目的　【日本語】
Ⅰ．在宅医療を継続する人とその家族のニーズ、社会的背景を包括的に把握し、生涯にわたる健康生活の実現に向けて効果的な支援体制の形成と調整能力を養う。
Ⅱ．高齢者を中心として，対象者の生活を視野に入れたリハビリテーションの意義を理解し，生活の場に応じた健康的な生活を創出（planning）し，具体的な援助を行うための視点および能力を養う。
Ⅲ.長期的な健康問題をもつ対象者の現状と課題について理解する。

The aim of this course is to
1. help students study healthy lifestyles and support systems for patients who need home medical care, and their families.
2. help students comprehend the meaning of rehabilitation from the viewpoint of the whole life and needed supports, especially of the elderly.
3. help students understand characteristics of diagnoses and treatments for people with long-term disease.
