Goals of the Course(JPN) | | 超高齢社会である日本においては、住み慣れた地域で医療、看護等の包括的な支援・サービス提供体制（地域包括ケアシステム）の構築の重要性が再認識され、行政よりも推進されている。長期入院等による医療費高騰を抑制する最終手段としても注目されている。在宅医療では従来の医学に求められた生命予後延長や治癒率向上ではなく、患者さんのADL,満足度、QOL,さらには介護者の介護負担軽減等が求められる。１.在宅医療システムと訪問看護、家族介護、活用すべき社会資源、福祉機器などについて幅広く学ぶ。
２.介護保険制度における要支援患者/要介護患者に対する訪問看護制度やケアマネジメントについて理解を深 める。
３.現在の在宅看護医療制度における課題を理解し解決策を考える |

Goals of the Course | | Japan was rushed into a super-aged society. The importance of the building up the comprehensive supporting and servicing system including
medical treatment and nursing in the patients’ residential area (Community-based integrated care systems) is re-recognized. It's also watched
as the last means to control medical expenses due to long hospitalization et al.

２.介護保険制度における要支援患者/要介護患者に対する訪問看護制度やケアマネジメントについて理解を深める。
３.現在の在宅看護医療制度における課題を理解し解決策を考える
上記を踏まえて各学生が掲げたテーマに沿って研究し国際学会での発表及び英語論文として発表する |

Objectives of the Course | | We aim understanding the following items in this course.
1 Understanding the system of home care service and home nursing, social resources and welfare equipment.
2 Understanding the home-visit nursing and care management in long-term care insurance system.
3 Analyzing and pursuing the issues in present Japanese long-term care insurance system
Finally, each student present their achievement as the paper in international meeting and in esteemed journal.

授業の内容や構成
Course Content / Plan | | 個々で下記の点について文献検索、実地調査等を行い討論し検討した上で、各自の掲げるテーマに沿って研究を進め論文化する。
１.在宅看護を受ける患者層（多くは高齢者）の疾病を以下に分類し各々の在宅看護での留意点、目標を考える。
１）寝たきりの原因としての脳血管障害,骨折,認知症等、
２）生命予後を左右する悪性腫瘍,虚血性心疾患,脳血管障害,呼吸器疾患等,
３）以上の疾患の基礎疾患及び併発症である生活習慣病，特に，糖尿病,高血圧, 脂質異常症や老年症候群（骨粗しょう症,尿失禁,転倒等）
2. 在宅看護を受ける患者層及び介護者の抱える問題及び訪問看護への期待について考える。
1. Classifing the diseases in the patient receiving home-care services as follows and consider the attention points in the patient.
1) Cerebral vascular accidents, bone fracture and dementia as the cause of bedridden status.
2) Malignancy, ischemic heart disease, cerebral vascular accident and respiratory disease etc threatenting life prognosis.
3) The lifestyle-related disease underlying the above mentioned diseases, in particular, diabetes mellitus, hypertension, dyslipidemia and geriatric
syndrome (osteoporosis, urinary incontinence, fall etc.)
2. We think about the problems of patient receiving home-care services and the expectance for home visiting nursing. care. |
