Goals of the Course
The aim of this course is to acquire the necessary measurement skills, to plan the physical therapy programs based on the integration of the results, and to gain experience of physical therapy interventions under the guidance of the supervisor.

授業の内容や構成
Course Content / Plan
各実習施設での指導計画による実習をおこなう。
実習期間（予定）は以下の通りである。

第1クール：2026年9月14日（月）〜10月15日（木）
第2クール：2026年10月22日（木）〜11月27日（金）
履修条件・関連する科目
Course Prerequisites and Related Courses
4年次春学期終了時までに修得すべき専門系科目（専門基礎科目及び臨床実習IIIを含む専門科目）の必修の授業科目の単位を全て修得していることが履修条件である。（便覧参照）
成績評価の方法と基準
Course Evaluation Method and Criteria
全ての実習に参加することを原則とする。実習に関するポートフォーリオ(デイリーノート、チェックシート、振り返りシート、サブノートなど)および評定表(2施設分)、レポート（事前課題、症例レポート）、書類の提出状況、出欠状況(マイナス評価、遅刻など)を踏まえ、総合的にA＋からFの6段階で評価する
